PROVIDER ENROLLMENT PROCEDURES

The attachment below provides information on waiver services, licensure requirements and enrollment procedures. You should read this item first.

AGED/DISABLED ADULT MEDICAID WAIVER (ADA)

This waiver provides in-home services to consumers age 18+ who meet certain eligibility criteria and who would otherwise need nursing home care. Services may include, but are not limited to, personal care, homemaking, respite and home deliveblue meals:

In order to enroll in the ADA Medicaid Waiver program, you must obtain a provider ID number by completing the Medicaid Application Packet:
